We are delivering a comprehensive programme of reform in response to Fatal Accident Inquiry (FAI) recommendations, embedding systemic improvements across the SPS. 

Guided by principles of transparency, accountability, collaboration, and continuous learning, our Delivery Framework ensures that change is structured, sustainable, and person-centred. 

By organising work into strategic delivery themes we address root causes and strengthen safety and wellbeing throughout the prison system. This approach reflects our commitment to learning from FAIs, driving cultural change, and building a safer, more responsive service for individuals in custody, their families, and our staff.

Strategic Delivery Themes

To deliver FAI recommendations effectively, we have adopted a thematic approach that groups related actions into key areas of focus.

 

Each theme is led by a subject matter expert who drives delivery plans, collaborates across our organisation and with external partners, and embeds improvements into everyday practice.

Workforce support and training

Considering all aspects of staff training and development, to ensure the implementation of all Fatal Accident Inquiry recommendations.

Suicide prevention

Strengthening identification, support, and intervention strategies for those at risk of self-harm or suicide, with a focus on early prevention and continuity of care.

Information sharing and record keeping

Ensuring vital court information follows every young person into custody - for safer, more informed care.
